Reporting accidents or incidents

Organisations and facilities will all have an accident report book or form. Any incident or accident that occurs must be recorded in the book by the injured party and a witness. The books or forms may appear slightly differently for all organisations, but all include similar information regarding;

  • The casualty (name, address and contact number)
  • The person completing the report (name, address and contact number)
  • The date and time of the incident
  • When and where the incident happened
  • What happened and how it happened
  • If any first aid or treatment received by the casualty.
