Informed consent form and PAR-Q

An informed consent form tells the participant about what they are going to be involved in and if there are any risks. It must be read and signed by the participant that is completing the fitness test or training programme to ensure that they are aware of what they are going to be doing. The form will include:

  • Basic procedures involved in the testing/training
  • Description of any risks that may be involved
  • Description of benefits that may be involved
  • Date, name, address, D.O.B, signature of participant, and counter signature of trainer.

Physical Activity Readiness Questionnaire (PAR-Q)

The physical activity readiness questionnaire (PAR-Q) is a self-screening tool that can be used by anyone who is planning to start an exercise program. It is often used by fitness trainers or coaches to determine the safety or possible risk of exercising for an individual based upon their answers to specific health history questions.
